NB2290 – Trust Certification Form

The NB2290 form is a Trust Certification Form used for verifying the details of a trust when it is designated as the owner of a life insurance policy or annuity contract. Purpose & Use:

• Certifies the existence and legitimacy of a trust when it owns an insurance policy or annuity.
• Provides Allianz or other financial institutions with accurate trust details and trustee authorization.
• Helps confirm that the trust is operating within legal and contractual guidelines.

Key Requirements:

• The trust name, date of establishment, and trustee details must be accurately provided.
• Trustees must confirm their authority to manage the trust's assets, including insurance or annuity contracts.
• In certain cases, a copy of the signed and dated trust agreement may be required, particularly for applicants aged 65 or older.
